Root & Revive Health Co, LLC, a Florida limited liability company, doing business as Root + Revive Health Co (“Practice”), provides telehealth-based medical services to adults in states where its providers are licensed or registered.
These policies explain how our telehealth practice operates, how we protect your privacy, how payments and appointments work, and how prescriptions and lab testing are handled. Our goal is to provide safe, transparent, and clinically appropriate care.

Telehealth Informed Consent
The Practice provides medical services through telehealth, which uses secure electronic communication to allow licensed healthcare providers to evaluate, diagnose, and treat patients remotely.
By initiating care and using our services, you consent to receive medical services via telehealth.

- Nature of Telehealth: Telehealth allows care without an in-person visit but may not be appropriate for all conditions. If an in-person evaluation is required, you may be referred to a local provider, urgent care, or emergency services.

- Limitations: Telehealth has limitations, including the inability to perform certain physical examinations.
- Voluntary Participation: You may decline telehealth services or end a session at any time.

HIPAA Privacy Policy
The Practice complies with all applicable requirements of the Health Insurance Portability and Accountability Act (HIPAA).
How We Protect Your Information
- We collect only the information necessary to provide medical care and operate our practice.
- Your protected health information (PHI) is stored in a secure, HIPAA-compliant electronic medical record system.
- Access to PHI is limited to authorized personnel.
- Information is shared only as necessary for your care (such as with laboratories or pharmacies) or when required by law.
- You may request access to or copies of your medical records at any time.
- Records are retained for at least seven (7) years in accordance with federal and state regulations.
Data Security Policy
We use industry-standard administrative, physical, and technical safeguards to protect your information, including encryption, secure servers, access controls, and staff training on privacy and data security practices.
Financial Policy
The Practice is a cash-pay medical practice and does not participate with insurance plans or submit insurance claims.
Payment Terms
- Payment initiates your medical evaluation.
- Payment is required in advance for consultations, lab coordination, and prescribed medications.
- Appointments are scheduled only after required forms and lab results are received and reviewed.
- A valid credit or debit card must be kept on file.
Payments are processed securely through Stripe.
Refund, Cancellation & Reschedule Policy
- Consultation fees are non-refundable once a visit has occurred or if cancelled within 24 hours of the scheduled appointment.
- One (1) courtesy reschedule is permitted per consultation.
- Missed appointments (no-shows) are non-refundable and require payment of a new consultation fee.
- If the Practice cancels your appointment or determines you are not clinically appropriate prior to care being rendered, applicable fees will be refunded.
Initial Consultation
The initial consultation fee covers:
- Provider review of your intake forms and records
- Telehealth visit
- Clinical assessment and care planning
Laboratory testing and medications are billed separately.
Laboratory Testing Policy
- Initial care requires complete and clinically appropriate laboratory work.
- Patients may upload qualifying recent labs or complete the Practice’s recommended lab panel.
- Lab fees are refundable only if the specimen has not yet been collected.
- Once labs are drawn or processed, fees are non-refundable.
Prescription & Treatment Policy
The Practice provides adult medical care including:
- Medical weight management
- Hormone replacement therapy
- Primary care support and sick visits
Prescribing Standards
- Prescriptions are issued only when clinically appropriate.
- High-dose, cosmetic, or performance-enhancing use is not permitted.
- Follow-up visits and lab monitoring are required when indicated.
Pharmacies
- Compounded medications are coordinated through licensed partner pharmacies and paid through the Practice prior to shipment.
- Root + Revive Health Co. does not dispense, compound, or ship prescription medications. All prescriptions are dispensed and shipped exclusively by licensed partner pharmacies.
- Primary care and sick-visit prescriptions are sent to the pharmacy of your choice and paid for directly by you.
Refills require an active patient-provider relationship and may require follow-up evaluation.
Supplements
- Supplements are optional and not required as part of care.
- Any supplement recommendations are based on clinical judgment and lab findings, and you may purchase supplements from any reputable source.
- Supplements are not regulated by the FDA in the same way as prescription medications and are not intended to diagnose, treat, cure, or prevent disease.
Chargebacks & Billing Disputes
Please contact the Practice directly with billing questions. Initiating a chargeback without first contacting us may result in suspension or termination of services.
Record Retention
Medical records are maintained in compliance with federal and state law and retained for at least seven (7) years after the last date of service. Requests for records may be subject to reasonable administrative fees.
