Root & Revive Health Co, LLC operates as a telehealth medical practice. By initiating care, booking or scheduling a visit, enrolling in a membership, or otherwise using our Services, you agree to these Terms, our Privacy Policy, and all applicable clinical, financial, and consent policies.
1. Nature of services
The practice provides telehealth-based medical care including initial consultations, follow-up visits, primary care support, sick visits, hormone replacement therapy, and medical weight-management services with GLP-1 medications when appropriate.
2. Eligibility & location requirements
Patients must be physically located in states where our clinicians are licensed, provide accurate information, and consent to telehealth services. Telehealth services are legally deemed to occur at the patient's physical location.
3. Establishment of the provider-patient relationship
A provider-patient relationship requires completing intake forms, clinician review, a telehealth evaluation, and a clinical determination of appropriate care. Submitting forms, paying fees, enrolling in a membership, or purchasing Services does not guarantee treatment, prescriptions, or ongoing care.
4. Telehealth limitations
Patients acknowledge that telehealth has limitations compared to in-person care, relies on patient-reported information, may experience technical interruptions, and requires patients to ensure a private environment. The practice does not provide emergency or urgent in-person care.
5. Primary care support (limited scope)
Primary Care Support is not comprehensive primary care and does not replace an in-person primary care provider. Services include medication review, chronic-condition check-ins, lab review, and preventive guidance. Services exclude emergency care, physical examinations, unstable conditions, and extensive administrative forms.
6. Sick visits
Sick visits are appropriate for upper-respiratory symptoms, gastrointestinal complaints, rashes, and uncomplicated urinary symptoms. They are not appropriate for chest pain, shortness of breath, neurologic symptoms, severe abdominal pain, pediatric emergencies, or conditions requiring immediate evaluation.
7. Memberships & packages
Memberships may include follow-up visits, primary care support visits, sick visits, and care coordination. Memberships are not insurance and do not roll over unused visits unless explicitly stated.
8. Asynchronous communication & messaging
Secure messaging covers care coordination and non-urgent communication. It is not appropriate for emergencies, new or worsening symptoms, urgent medical decisions, or controlled-substance requests.
9. Prescriptions & medications
Prescriptions are issued only when clinically appropriate and lawful. The practice does not guarantee that any medication will be prescribed; continued prescribing if required labs, follow-ups, or monitoring are not completed; or medication availability or pricing.
10. Laboratory testing & monitoring
Certain treatments require lab testing. Patients must complete required labs within specified timeframes and pay applicable fees. Failure to complete testing may result in care interruption.
11. Payment, billing & financial responsibility
Patients are financially responsible for all services. Payment initiates your medical evaluation and is due before or at the time of service. The practice does not guarantee insurance reimbursement.
12. Cancellation, no-show & refund policy
Appointments must be cancelled 24 hours in advance. One courtesy reschedule is permitted per consultation. Late cancellations and no-shows are non-refundable. Completed clinical services are non-refundable.
13. Emergency care disclaimer
The practice does not provide emergency services. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.
14. User responsibilities
Patients must provide accurate information, maintain account security, use services lawfully, and not record visits without consent. Abusive, fraudulent, or inappropriate behavior may result in termination of care.
15. Privacy & HIPAA
The practice complies with HIPAA and state privacy laws. Information may be shared with labs, pharmacies, payment processors, and vendors as necessary.
16. Intellectual property
Website content and materials are the property of the practice and cannot be reproduced without permission.
17. Modification or termination of services
The practice reserves the right to modify, suspend, or terminate services for clinical, legal, or operational reasons.
18. Governing law
These Terms are governed by the laws of the State of Florida.
