Terms of Service

Root + Revive Health Co. — Terms of Service

Root & Revive Health Co, LLC, a Florida limited liability company, doing business as Root + Revive Health Co (“Practice,” “we,” “us,” or “our”), provides telehealth-based medical services. These Terms of Service (“Terms”) govern your access to and use of our websites, telehealth services, memberships, subscriptions, clinical programs, and digital platforms (collectively, the “Services”).

By initiating care, booking or scheduling a visit, enrolling in a membership, or otherwise using our Services, you agree to these Terms, our Privacy Policy, and all applicable clinical, financial, and consent policies.

1. Nature of Services

The Practice is a provider-owned medical practice offering telehealth-based care, including but not limited to:
Initial medical consultations
Follow-up visits
Primary care support (limited scope)
Sick visits for acute, non-emergent conditions
Hormone replacement therapy (HRT)
Medical weight-management services (including GLP-1 medications when clinically appropriate)

Services are provided via secure telehealth platforms and are subject to clinical judgment, licensure requirements, and applicable state and federal law.

2. Eligibility & Location Requirements

You may receive Services only if:
You are physically located in a state where our clinicians are licensed or legally authorized to provide telehealth services;
You provide accurate, complete, and truthful medical and demographic information; and
You consent to telehealth services and all applicable treatment-specific consents.Telehealth services are legally deemed to occur at the patient’s physical location. You are responsible for confirming your location at each encounter. The Practice reserves the right to decline or discontinue Services if eligibility requirements are not met.

3. Establishment of the Provider-Patient Relationship

A provider-patient relationship is established only after:
Completion of required intake forms and consents;
Review by a licensed clinician;
A telehealth evaluation; and
A clinical determination that care is appropriate.
Submitting forms, paying fees, enrolling in a membership, or purchasing Services does not guarantee treatment, prescriptions, or ongoing care.

4. Telehealth Limitations

By using the Services, you acknowledge that:
Telehealth has inherent limitations compared to in-person care;
Providers rely in part on patient-reported information;
Technical issues may occasionally interrupt care; and
You are responsible for ensuring a private, secure environment during visits.
The Practice does not provide emergency or in-person urgent care.

5. Primary Care Support (Limited Scope)

Primary Care Support is not comprehensive primary care and does not replace an in-person primary care provider.
Primary Care Support may include:
Medication review and refills (when clinically appropriate)
Stable chronic condition check-ins
Lab review and interpretation
Preventive care guidance
Sleep, stress, and lifestyle counseling
Primary Care Support does not include:
Emergency or urgent care
In-person physical examinations
Management of unstable or complex conditions
Hospital follow-up care
Disability paperwork or extensive administrative forms
Same-day urgent evaluations outside defined sick-visit parameters
The Practice may refer you to in-person or specialty care when clinically indicated.

6. Sick Visits
Sick visits are intended for acute, non-emergent conditions such as:
Upper respiratory symptoms
Gastrointestinal complaints
Rashes or minor skin concerns
Uncomplicated urinary symptoms

Sick visits are not appropriate for:
Chest pain
Shortness of breath
Neurologic symptoms
Severe abdominal pain
Pediatric emergencies
Any condition requiring immediate in-person evaluation

If symptoms suggest an emergency, you will be directed to seek in-person or emergency care.

7. Memberships & Packages
The Practice may offer memberships that include defined services over a stated period, which may include:
follow-up visits
primary care support visits
sick visits
Access to care coordination or secure messaging

Memberships:
Are not insurance
Do not override clinical judgment
Follow‑up and sick visits are included but subject to reasonable usage limits
Do not roll over unused visits unless explicitly stated

8. Asynchronous Communication & Messaging

Secure messaging may be offered for care coordination and non-urgent communication, including:
Clarifying treatment plans
Non-urgent follow-up questions
Administrative guidance

Messaging is not appropriate for:
Emergencies
New or worsening symptoms
Urgent medical decisions
Requests for controlled substances

Providers may require a scheduled visit to address medical concerns.

9. Prescriptions & Medications
Prescriptions are issued only when clinically appropriate, lawful in the patient’s state, and supported by adequate evaluation.
The Practice does not guarantee:
That any medication will be prescribed;
Continued prescribing if required labs, follow-ups, or monitoring are not completed; or
Medication availability or pricing.

Controlled substances are prescribed only in compliance with DEA and state regulations. Failure to complete required monitoring may result in suspension or discontinuation of prescriptions.

10. Laboratory Testing & Monitoring
Certain treatments require laboratory testing and ongoing monitoring. You agree to:
Complete required labs within the specified timeframe;
Use approved lab partners when required; and
Pay applicable lab fees.

Failure to complete required testing may result in interruption or discontinuation of care.

11. Payment, Billing & Financial Responsibility
You are financially responsible for all Services provided.
Payment initiates your medical evaluation.
Payment is due prior to or at the time Services are rendered.
Membership fees, if applicable, are billed on a recurring basis. You authorize the Practice to charge the payment method on file. The Practice is a cash-pay practice and does not guarantee insurance reimbursement.

12. Cancellation, No-Show & Refund Policy
Appointments must be cancelled or rescheduled at least 24 hours in advance.
One (1) courtesy reschedule is permitted per consultation.
Late cancellations and no-shows are non-refundable.
Completed clinical services are non-refundable.
Medication fees are non-refundable once processing or fulfillment has begun.
Repeated no-shows may result in termination of Services.

13. Emergency Care Disclaimer
The Practice does not provide emergency medical services.
If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.

14. User Responsibilities
You agree to:
Provide accurate and complete information;
Maintain the security of your account and login credentials;
Use Services respectfully and lawfully; and
Not record visits without express consent.
Abusive, fraudulent, or inappropriate behavior may result in termination of care.

15. Privacy & HIPAA
The Practice complies with HIPAA and applicable state privacy laws. Your information may be shared with laboratories, pharmacies, payment processors, and technology vendors only as necessary to provide care and operate the Practice.

16. Intellectual Property
All website content, branding, logos, and materials are the property of the Practice and may not be reproduced or used without written permission.

17. Modification or Termination of ServicesThe Practice reserves the right to modify, suspend, or terminate Services at any time for clinical, legal, or operational reasons.

18. Governing Law
These Terms are governed by the laws of the State of Florida, without regard to conflict-of-law principles.
